Inside the Audit Bundle: What Ships in a Signed WoundScribe Encounter
product
A breakdown of the WoundScribe audit bundle — final notes, patient summary, revision trail, signatures, compliance results, and billing status in one downloadable package.
When a WoundScribe encounter is signed, the platform assembles an audit bundle: a single downloadable package built for payer requests, internal QA, and legal discovery. Here's what's inside and why each artifact matters.
The seven artifacts
| Artifact | What it contains | Why it matters |
|---|---|---|
| Final SOAP note | Signed Subjective, Objective, Assessment, Plan | The clinical record of the encounter |
| Patient summary | Plain-language visit recap and instructions | Patient education and continuity |
| Revision trail | Every edit between draft and signature, timestamped | Demonstrates integrity of the note |
| Signatures | Provider signature, addenda, co-signs | Attestation and accountability |
| Compliance results | Pre-sign flags, resolutions, and acknowledgments | Proves due diligence on payer rules |
| Billing status | E/M level, CPT, HCPCS, ICD-10, modifiers | Ties documentation to claim |
| Imaging package | Wound photos with calibration-free measurements | Defensible visual baseline |
What makes it audit-ready
- Timestamped revision trail. Auditors can see the note wasn't back-dated or reconstructed. Each agent draft, human edit, and sign-off event is logged.
- Compliance results are preserved, not discarded. If Ask Pingoo flagged a missing modifier 25 and you added it, the flag and resolution both appear.
- Imaging is bound to measurements. Photos ship with the length, width, and depth values that drove the CPT selection, along with the capture device and timestamp. See audit-ready wound photos for what defensible imaging requires.
- Deliberate unlock for addenda. The chart locks at signature. Reopening creates a new signed addendum rather than overwriting the record.
