Wound imaging is where most documentation systems either over-promise or under-deliver. The WoundScribe Wound Imaging Agent is built to do three jobs well and show its work for each one.

Tissue composition

The agent segments the wound bed into granulation, slough, eschar, and epithelial tissue, expressed as percentages. Composition is tracked across visits so trends are visible at a glance — not buried in narrative.

Measurement

Length, width, depth, and area are computed from the captured image with reference scale. Measurements are versioned per visit, with the prior values shown for context. When confidence is low, the agent surfaces that rather than guessing.

Peri-wound tracking

Maceration, induration, erythema, and callus around the wound margin are flagged and trended. This matters for diabetic foot ulcers and venous wounds where peri-wound status often predicts the next complication.

Where it fits in the encounter

Imaging output feeds the assessment and plan drafts, so the clinician is not retyping measurements into a note. It also informs the coding suggestion — wound size influences debridement CPT selection.
