Non-affirmation isn't a coverage denial. It's usually a documentation gap CMS's reviewer couldn't fill in. These are the questions wound care clinics ask most once the first rejection lands.

What does non-affirmation actually mean?

It means the reviewer couldn't confirm medical necessity from what you submitted. Coverage rules haven't changed — the evidence didn't clear the bar. You can resubmit with additional documentation.

What's the most common reason skin substitute requests get non-affirmed?

Templated rationale. Notes that could describe any patient with any wound. Reviewers want specifics: ulcer location and laterality, exact measurements, duration, prior treatments tried, and why this product at these units.

Do I need to show conservative care every time?

Yes, for chronic wound skin substitutes. Most LCDs require documentation of at least four weeks of standard care — offloading, debridement, moisture management, glycemic or vascular optimization — with no meaningful healing.

How many measurements do I need?

At minimum two dated measurements showing the wound has stalled or regressed. Photos with a reference marker help. A single measurement at the visit isn't enough.
