Why a Callus Is a Warning: Pressure, Sensation Loss, and Hidden Wounds

A closer look at the diabetic foot callus — why it forms, why it's dangerous when sensation is reduced, and why you should never trim one yourself.

To most people, a callus is just thick skin. To someone with diabetes, it's a warning sign that pressure is building in a spot where a wound may already be forming underneath.

How a callus forms

Skin responds to repeated pressure or friction by thickening. Under normal conditions, that's protective. Under diabetic conditions — where nerves may have lost the ability to signal pain — it becomes a hazard, because:

  1. You can't feel the pressure that's causing it
  2. The callus itself increases pressure on the tissue beneath
  3. Blood flow to that spot may already be reduced
  4. Small blood vessels can break under the callus, forming a hidden hematoma
  5. Skin under the callus can break down into an ulcer that's invisible from the surface

By the time you see drainage seeping through the callus, there's often already an established wound underneath.

The physics: pressure and shear

Calluses form where two forces concentrate:

  • Vertical pressure — the plantar surface of the metatarsal heads, the heel, the tip of a hammer toe
  • Shear — sideways friction from a shoe that doesn't fit, or from a gait change caused by pain elsewhere

Common high-risk locations include the ball of the foot under the first and fifth metatarsals, the tips of clawed or hammered toes, and the medial side of the great toe.

Why you should never trim a callus at home

  • You can't feel how deep you're cutting
  • The tissue beneath may already be compromised
  • A small nick becomes an entry point for bacteria
  • Home tools aren't sterile

Callus care belongs in the hands of a podiatrist or wound care clinician who can safely debride the thickened tissue and inspect what's underneath.

What actually solves the problem

Removing the callus doesn't fix why it formed. Real prevention addresses the pressure:

  • Offloading footwear — diabetic shoes, custom insoles, or a total contact cast for active ulcers
  • Toe deformity correction — sometimes surgical, sometimes with padding and splinting
  • Daily foot inspection so you catch the next callus before it thickens
  • Blood sugar control so the underlying nerve and vessel damage doesn't progress

What to do this week

Inspect your feet today. If you find a callus — especially one that's yellow, has a dark spot in the center, or feels warm — book an appointment. Don't wait for it to hurt. It probably won't.
