What lands on the superbill

Every superbill assembles four required blocks: patient and insurance demographics, the diagnosis list in payer-preferred order, each CPT with its modifiers and diagnosis pointers, and the signed provider attestation with credentials and NPI.

How linked diagnoses carry through

Because diagnoses are linked to CPTs during coding, the superbill inherits the pointers automatically. A 97606 tied to E11.621 and L97.421 shows up on the printout with those Dx already pointed — no re-keying, no missed links.

Modifier handling

Laterality (RT/LT/T-codes), modifier 25 for separately identifiable E/M, and modifier 59/XS for distinct procedural services are applied at coding time and printed on the correct line. A same-day E/M + debridement + NPWT visit prints as three lines with the right modifiers stacked.

The provider attestation
