Is a Simple Note audit-defensible?

Yes. Each Simple Note is a signed SOAP note stored in the AI-powered EMR for wound care, timestamped, and attached to the patient record. It carries the subjective, objective, assessment, and plan structure auditors expect for a low-complexity encounter.

What counts as a "simple" encounter?

Refill calls, telehealth check-ins, triage phone calls, and brief post-op confirmations. Anything without a physical exam, wound imaging, measurement, or procedure. If you're capturing wound images or coding an office visit, use a Full Encounter instead.

Can Simple Notes be billed?
