Referrals fail quietly. A fax goes out, no one confirms receipt, the patient never gets a call, and the loop stays open for months. The referral lifecycle bar exists to make that impossible.

The five states

  1. Sent — order transmitted to the specialist (fax, direct, or portal)
  2. Acknowledged — receiving office confirmed receipt
  3. Scheduled — appointment booked with a date
  4. Seen — visit completed, consult note received
  5. Closed — recommendations reviewed and integrated into the plan

What advances a state

States advance automatically when signals arrive — a fax confirmation, a scheduling call logged in Updates, a consult note attached to the encounter. Clinicians can also manually Advance to Seen or jump to any state and set the appointment date directly.

Every call, fax, and payer interaction is logged alongside the referral: prior auth approved by Noridian, specialty office confirming the July 1 appointment, consult report received. The timeline is the audit trail auditors and care coordinators both need.

Prior auth visibility inside the lifecycle

For referrals requiring authorization, the lifecycle bar shows auth status inline: pending, approved (with auth number and expiration), or denied. Vascular referrals driven by ABI findings, for instance, show AUTH-58302 with expiration and the approval letter attached.

Why loop closure matters

CMS and Joint Commission both hold the ordering provider responsible for confirming referrals reach completion. Open loops are a documented patient safety risk and an audit exposure. Closed-loop tracking pairs naturally with WISER claims and compliance.
