Vendors love accuracy demos. Practices need production metrics. Two numbers tell you whether an AI-drafted superbill is actually working in your clinic: clinician override rate and claim denial rate. Everything else is decoration.

Clinician override rate

Defined as the share of AI-drafted lines that the clinician edits, removes, or replaces before approving the superbill. A healthy workflow shows:

  • Week 1 — 20–35% override rate as the model calibrates to clinician phrasing and local patterns.
  • Week 4 — under 10% for routine visits, higher only on genuinely ambiguous cases.
  • Steady state — override rate stable and traceable to specific categories (e.g., E&M leveling), not scattered.

Override rate is diagnostic. A spike in one CPT family points at a rule to tighten. A spike from one clinician points at a documentation-habit tweak, not a model failure.

Claim denial rate

Defined as the share of submitted claims returned unpaid or pended. In wound care, common causes are:

  • Missing or misapplied modifiers (25, 59).
  • Surface area or depth documentation not supporting the billed tier.
  • Skin substitute claims missing prior conservative care evidence.
  • LCD or NCD mismatches.
