Offloading a Diabetic Foot Ulcer: Why Pressure Relief Determines Healing

You can change dressings perfectly and still watch an ulcer fail. The reason is almost always pressure. Offloading — removing weight from the wound — is the single most important factor in whether a diabetic foot ulcer closes.
Why offloading matters
Every step on an ulcerated foot delivers force many times body weight to a tiny area of already-damaged tissue. That pressure crushes new capillaries, tears fresh granulation tissue, and pushes bacteria deeper. Neuropathy removes the pain that would normally stop you, so you keep walking and the wound keeps reopening.
The main offloading options
- Total contact cast (TCC). The clinical gold standard for plantar (bottom-of-foot) ulcers. A fiberglass cast spreads load across the entire lower leg. It cannot be removed by the patient, which is why it works — but it requires weekly clinic visits.
- Removable cast walker (boot). Nearly as effective as a TCC if worn continuously. Compliance is the limiting factor; some clinicians wrap the boot to make it non-removable.
- Surgical shoe or post-op shoe. Lower protection, used for smaller or healing ulcers, or when a boot is not tolerated.
- Felted foam or custom insoles. Adjunct pressure redistribution for specific ulcer locations.
- Wheelchair or crutches. Full non-weight-bearing when nothing else fits the anatomy.
What patients get wrong
- Taking the boot off at home 'just to walk to the bathroom.' Even a few steps a day can stall healing.
- Wearing the boot without socks or over a bulky dressing that changes fit.
- Skipping the boot at night when getting up.
What clinicians track
Offloading effectiveness shows up in the wound trend. A stalled or worsening trajectory despite good dressing care is a signal to escalate offloading — from shoe to boot, or boot to cast.
