Markerless wound imaging is the single feature that separates modern wound care software from digital paper. Here's what it actually does, and why the bedside workflow changes when you have it.

The problem with stickers, rulers, and reference cards

Traditional wound photography needs a fiducial — a printed sticker, adhesive ruler, or reference card placed near the wound so the software can scale the pixels. That sounds minor until you count the failure modes:

  • The sticker peels off in wound exudate
  • The ruler shadows the wound bed and corrupts tissue analysis
  • The clinician forgets it in the drawer on a home visit
  • Infection control policies restrict adhesives on some patients

Every missed sticker means a measurement you cannot defend in audit.

How markerless capture works

Markerless imaging uses the phone's own sensors — camera intrinsics, depth data, and motion — to build a 3D reconstruction of the wound surface. From that reconstruction the software derives:

  • Area — perimeter traced automatically, in square centimeters
  • Depth — measured from the wound margin, not eyeballed
  • Tissue composition — percentages of granulation, slough, eschar, and epithelial tissue
  • Volume — for cavity wounds and tunneling assessment

No sticker enters the frame. The clinician points, captures, and moves on.

What changes at the bedside

Capture time drops from roughly 90 seconds per wound (position sticker, photograph, retrieve sticker, discard) to under 15 seconds. On a caseload of 20 wounds a day, that is 25 minutes returned to patient care.

More importantly, measurements become consistent across clinicians. Two nurses measuring the same wound with paper rulers routinely disagree by 20% or more; a markerless system removes the human error from the scaling step.

What to test before you buy

Bring a hard case: a heel ulcer with undermining, a sacral pressure injury with irregular geometry, or a diabetic foot ulcer with slough at the base. Then check:

  1. Does the capture succeed on the first try, or does the app ask for a retake?
  2. Is the perimeter trace accurate, or does it wander into peri-wound skin?
  3. Are tissue percentages defensible against your own clinical read?
  4. Does the measurement flow directly into the signed note and the healing analytics dashboard?

If any of those fail, the imaging is a demo feature, not a bedside tool.
