Insulin absorption isn't uniform across the body, and for a patient with an active diabetic foot ulcer, site choice quietly shapes how steady their glucose stays between visits.
Absorption varies by site
Subcutaneous insulin enters the bloodstream at different rates depending on where it's injected:
- Abdomen — fastest and most consistent absorption
- Outer arm — moderate
- Outer thigh — slower
- Outer gluteus — slowest
Rotating within one anatomical region (rather than jumping between regions dose to dose) gives more predictable glucose curves — which matters when a wound is trying to close.
Why the foot and lower leg are off-limits
Never inject insulin into or near the ulcer, into edematous tissue, or into a limb with compromised perfusion. Injection trauma, altered absorption, and infection risk all work against healing.
Lipohypertrophy: the silent absorption problem
Repeated injections in the same spot create firm, rubbery lumps under the skin. Insulin injected into lipohypertrophy absorbs erratically — patients often present with unexplained hyperglycemia despite "perfect" adherence. Palpate injection sites at every DFU visit and document what you find.
