Xerosis — abnormally dry skin — affects the majority of patients with long-standing diabetes. Autonomic neuropathy reduces sweat gland output, and the resulting cracks are direct portals for bacteria. Daily moisturizing is a prevention step, not a cosmetic one.
Which ingredients actually help
- Urea (10–25%) — humectant and mild keratolytic; excellent for heel fissures.
- Ammonium lactate (12%) — alpha hydroxy acid that hydrates and thins hyperkeratotic skin.
- Petrolatum-based emollients — occlusive; best applied over a humectant to lock in water.
- Glycerin — humectant, well tolerated in lighter formulations.
Avoid heavily perfumed lotions, alcohol-based products, and anything marketed as 'antibacterial' for daily use.
Where to apply
- Tops of the feet
- Soles and heels — the highest-yield location for fissure prevention
- Around (but not between) the toes
- Up the ankle and lower leg if dryness extends
Where NOT to apply
Never between the toes. The interdigital spaces stay warm and moist by default. Adding cream traps moisture, macerates skin, and creates ideal conditions for tinea pedis and bacterial superinfection — both of which are common precursors to a diabetic foot ulcer.
Technique that actually works
- Moisturize immediately after washing and drying, while skin is still slightly damp.
- Use a small amount — a pea-sized dab per foot is usually enough.
- Massage in gently; do not scrub cracked heels.
- Wipe any excess from between the toes with a dry towel.

Include a skin-integrity assessment at every visit and document xerosis, fissures, and interdigital maceration as separate findings.
