How to Finish Wound Care Notes Before You Leave the Room
documentation
A step-by-step workflow for charting wound visits at the bedside, so notes are done before the next patient and your evenings stay free.
Most wound care notes get written late because the details are still in your head, not in the chart. Moving documentation into the visit itself fixes that. Here is a simple workflow you can try without changing your EMR.
Before you start
- Open the visit on iPhone, iPad, or web.
- Confirm the patient and the wound sites you expect to see.
- Tell the patient you will be documenting as you talk.
The in-room workflow
- Capture the wound. Take the image at the bedside so measurements and photos live with the visit. See how bedside imaging and mobile charting work.
- Talk through the exam. Describe findings, drainage, and plan the way you normally would. An AI scribe for wound care drafts from that conversation.
- Review the draft. Read the SOAP note, edit what needs editing, and confirm the details match what you saw.
- Sign before you leave. The note is done while the visit is fresh.
- Move it to your EMR. Keep your current system as the record of care.
Why this order works
Capturing first gives you objective data. Talking second gives the scribe context. Reviewing last keeps you in control of what gets signed. For more on the drafting step, read how AI drafting turns conversation into SOAP notes.
Start small
Pick one visit tomorrow and chart it this way. Compare the time to your usual routine, then decide what to do next.
