Ambient AI Scribe for Wound Visits: How the Agent Drafts Your SOAP Note
product
A deep dive into the ambient scribe that turns a wound care conversation into a signed SOAP note — what it hears, how it structures, and where it hands off.
Most ambient scribes were built for primary care visits. Wound care has its own vocabulary — undermining, tunneling, slough, eschar, Wagner grades — and its own documentation requirements for Medicare. Here's how the wound-tuned ambient scribe works under the hood.
What the scribe listens for
The agent is trained on wound care encounters, not general medicine. It recognizes:
- Wound etiology terms (DFU, VLU, pressure injury, arterial)
- Staging and classification language (Wagner, University of Texas, NPUAP stages)
- Vascular exam phrases (palpable DP/PT, monofilament testing, ABI results)
- Procedure cues (selective debridement, sharp debridement, offloading)
How it structures the note
The conversation is parsed into SOAP:
- Subjective — pain, duration, prior treatment, patient's own story
- Objective — wound bed, periwound, exudate, vascular findings, measurements from Point-n-Capture
- Assessment — etiology and staging the clinician confirms
- Plan — procedures performed, dressings ordered, follow-up, referrals
Handoffs to other agents
The scribe doesn't code the visit alone. It passes structured data to:
- The imaging agent for measurements
- The coding agent for E/M level, CPT, and ICD-10
- WISER claims and compliance for pre-submission validation
- The healing dashboard for trajectory tracking
What the clinician still does
Sign. The AI scribe for wound care drafts; the clinician reviews etiology, confirms the plan, and signs. The AI does not auto-submit claims or finalize notes.
